Fill out the necessary information on the application form and prepare the following 4 points.
① Admission fee 2,000 yen (only for first time)
② Annual fee 5,000 yen (once a year)
③ Monthly membership fee (monthly)
④ Insurance premium 1,800 yen (valid from the following month of the payment month until the next March 31st)
- Discount service ☆
- Family discount ① Admission fee, annual fee will be charged at half price. (From the second person onwards)
② Monthly membership fee will be discounted 1,000 yen
